Coaching Agreement with Dr. Rebecca Cardona, Ph.D.

Welcome!

As the life coach, I look forward to partnering with you as a personal growth & spiritual life coach to provide the life coaching services I describe below. My coaching method is based on the premise that you have the answers and the information you are  looking for; I am simply here to help you find those
answers and the clarity within yourself.
Please review all aspects of this Agreement, fill out the bottom portion, scan both pages and send the signed Agreement back to me at  drcardona@fearnothingcoaching.com. If you have any questions or concerns about this Agreement, please contact me prior to signing at 865-387-7500.

About Me:

1. I received life coach training from a life coach who graduated from the Martha Beck Institute.

2. I hold a Bachelor’s degree in academic psychology, a Master’s degree in mental health, marriage/family counseling and a Ph. D. degree in Philosophy with a specialization in marriage, family, couple counseling.

3. Before becoming a personal growth and spiritual life coach, I worked as a vice president, director and manager for 15+ years in behavioral health, strategizing, managing, coaching, developing, and helping to grow others and opportunities both personally and professionally.

4. I adhere to a strict code of professional standards.

 

What you can expect from me. I will…

1. Be a partner in bringing out the best, the deepest  and the truest in you.

2. Provide a safe and supportive environment in which you can relax and explore.

3. Help expand your view of what is possible and promote discovery of new insights.

4. Give you input, direct feedback, challenge you, and operate as a sounding board.

5. Listen carefully to what you say and ask questions that increase awareness.

6. Be an on-going resource for you in accomplishing your intentions.

7. Keep all your information and records confidential unless otherwise required by law.

I expect that you, as my client, will:

1. Cultivate a core honesty with yourself.

2. Take responsibility for creating value and results for yourself.

3. Be open to my feedback and keep me honestly informed as to what is and is not working for you.

4. Take ownership for your progress and your accomplishments.

5. Take full responsibility for the decisions and actions you take regarding your life.

6. Be mindful of your physical, mental and emotional well-being during the course of this coaching process and seek professional mental health treatment or other support services if appropriate for your situation.

About Our Sessions:

1. Sessions are 60 minutes long.

2. You have the option of in-person sessions (if you live in East TN) or a phone session or video session.

3. In-person sessions are held at my home office: 150 Hartford Village Way, Kingston, TN 37763.

4. Phone or video sessions: I will call or invite you to a video conference using Zoom, whichever we decide.

5. You agree to pay $100.00 per coaching session.

6. Payment for each session is due before the session begins. I accept debit and credit cards via PayPal.

7. The rate per session is subject to adjustment from time-to-time. Any changes in rates are applied prospectively, and I will provide you with at least 30 days advanced notice before changing my rate. The rate contained in any such notice shall become a term of this Agreement supersede and replace the rate provided in this paragraph.

8. You must provide at least 24 hours prior notice if you need to cancel or reschedule a session. If you fail to provide such notice, you are responsible for payment for the session.

9. If you arrive late for a session, the session will not be extended and will conclude at its previously-scheduled end.

 

By electronically signing below, I hereby certify that I have read the terms of this Agreement and agree to be bound thereby.
